Groan... I had a lousy lunch! 
I'm really lucky in that there's this fantastic salad bar cafe on the main floor of my building. I create some wonderful veggie-salads with tuna or ham (what ever meat they have stocked that catches my eye) and a sprinkle of cheese (tossed in my own dressing - don't trust what they have) and a couple of mega-spicy-olives. MMM! Sometimes I bring my own meat from home, but usually I opt for the tuna they have at the bar. I'm really happy with this lunch and can eat it often!
Well... today I left lunch a little late... busy with a project on my computer.
I arrive at the cafe to see that there is NO green letuce left! I was hungry and didn't have my problem-solving hat on. Rather then opt for some yummy green beans and other legal veggies - and taking a quick trip to another store in our lobby for a small tin of flavoured tuna (i.e., lemon pepper - yum!) to go with it - I panicked!!
What did I do? I went across the street to the hospital cafeteria to get something from their salad bar. What a disappointment!! Tossed salad, black olives and a little cheese... that's all they had that I could eat! And no meat available like at the cafe I go to. So... I go to the grill and ask for a burger (no bun, obviously).
With food in hand I make my way back to my office. One bite of the burger patty... cardboard! I even tried mixing it in with my salad so it could get a little dressing on it, but that didn't help. The rest of the burger I smothered in mayo just to make it palatable.
But that's not the worst... I was obviously so anxious (and hungry!) to get food that I didn't even notice until I was HALF WAY THROUGH EATING(!!!) that the mixed greens I got from the stupid cafeteria had... shreaded carrots in it! Damn! Well... not enough carrots to dump me out of ketosis, but enough for me to add an additional carb (maybe two) to today's count. Double Damn!
Anyway... lessons:
1) Don't take lunch so late that the salad is gone from the cafe
2) Have a back-up plan that doesn't involve going to the hospital cafeteria!
I'm sure there are others, but that's all I can think of at the moment
Anyway... dinner will be scrumptous. We're having pork marinated in garlic and rosemary with a tossed salad and avocado! That'll make up for my disasterous lunch!
