What happens if during owl I slip out of ketosis? Should I go back to induction foods until I get back into ketosis and then restart owl? Or should I go back to induction foods then start back with owl where left off excluding the foods that took me out of ketosis?

So adding 5 net carbs of more Induction veggies shouldn't throw you out of ketosis.
The stix might stop registering when you go over 40 net carbs daily. But as long as you still have the physical signs of ketosis and are still losing weight, you won't have to worry so much about "falling out of ketosis".
Generally, if you have a slip up during OWL. Let's say you accidentally mis-measure and end up eating a total of 50 net carbs for a day rather than 30. All you have to do is pick up where you left off. No need to "restart Induction" or "go back" to Induction.
You only need to go back to Induction if you completely blow it and go on a carb binge for a couple of days. At that point, you only go back until you get back into ketosis and then you pick up where you left off.
